New IRS Form for Some Determination Requests

The IRS has announced a new form, Form 8940, for tax-exempt organizations to use in order to request certain determinations. The form can be used to request the later determinations: advance set-up your sleeve approval; advance approval of voter registration activities; advance approval of scholarship procedures; immunity from Form 990 filing requirements; advance approval of "unusual grant" classification; exchange in type (or initial determination of type) of a section 509(a)(3) supporting organization; reclassification of foundation reputation; and termination of private foundation reputation. The form's instructions specify what additional information must be submitted to support each of the requests.
